The Card states, “Whilst carrying out a practice supply dropping detail over Seletar airfield, A made one without incident. Whilst it was turning port in towards airfield for a live drop, it was seen to lose height and crash in a nose down attitude with port wing low. All occupants were killed.”
“AIB found that while No2 E was stopped with prop fully feathered, when A struck G nos 1,3 and 4 Es were still operating. No reason to account for the feathering was established. The most probable Prime cause was loss of control by P whilst turning on asymmetric power, consequently A stalled and crashed. A conrib cause was the possible failure of no 2 E.
